Our Principle
Families deserve free access to the same quality data regulators have. We don't rank facilities with a made-up score, and we don't sell your information to placement agencies. We reproduce the official CMS record, add peer context, and link you straight back to the source.
Data Sources
CMS Provider Data Catalog
Published by the Centers for Medicare & Medicaid Services (CMS). This is the same data that powers Medicare's public Care Compare site. We use the nursing-home Provider Information, Health Deficiencies, and Penalties datasets, joined on each facility's CMS Certification Number (CCN). Refreshed roughly monthly.

Medicare Care Compare
The authoritative, live record for each facility. Every profile links to its Care Compare page so you can confirm the current data directly with Medicare.

Hospitals, Home Health, Hospice & Dialysis (Coming Soon)
FacilityProof starts with nursing homes — the richest red-flag story. The same CMS pipeline extends to hospitals, home-health agencies, hospice providers, and dialysis centers, which we're adding next.
What We Verify
- CMS certified & active: The facility is Medicare/Medicaid certified
- Overall rating 3+ stars: Its CMS overall Five-Star rating is average or better
- Staffing at/above state average: Nurse hours per resident per day meet or beat the state norm
- No severe deficiencies: No actual-harm or immediate-jeopardy findings in the latest survey
- No recent penalty: No recent federal fines or payment denials
How the Trust Score Works
Our 0–100 trust score re-expresses CMS's own signals: 40% overall star rating, 25% staffing versus the state average, 20% deficiency severity, and 15% enforcement history. Facilities are placed in tiers by percentile within their own city and facility type, so “Top Rated” is always relative and defensible. When a facility type lacks a signal (for example, home-health agencies have no staffing-hours field), we redistribute that weight rather than invent a value.
What We Don't Do
- We don't invent star ratings — they're CMS's relative measure
- We don't sell leads or gate the honest data behind a phone call
- We don't show live ER wait times — CMS timing measures are historical averages
- We don't provide medical advice or replace a personal visit
